Adult Social Work Officer, Haringey  
To strengthen the Hospital Discharge Team and ensure timely, safe, and effective discharges, I propose employing two locum workers in the role of Social Work Officer (akin to Assistant Social Worker). These positions will provide critical support in a fast-paced environment, working collaboratively with social workers, health professionals, and discharge coordinators. Key Purpose of the Role: Support hospital discharge functions by facilitating assessments, care planning, and coordination of services. Maintain a physical presence across key hospital sites, including: Whittington Hospital Royal Free Hospital - North Middlesex Hospital Rehabilitation beds under Pathway 2 Out-of-borough hospitals as required Assist in managing complex discharge pathways, ensuring compliance with statutory duties and local protocols. Contribute to reducing delayed transfers of care and improving patient flow. 
£29.25 per hour
